Chef card

An allergy chef card for UK restaurants

Anyone can look at what the SafePlate chef card looks like. The sample below uses peanut, which can cause anaphylaxis, and a severe mustard allergy, in English and in French. Filling in your own foods, then showing the card or printing it, needs SafePlate Plus. Search stays free.

Chris Faulkes, who is anaphylactic to peanuts and severely allergic to mustard, built SafePlate for that conversation with a kitchen. The sample is not his personal card, and it is not a promise that a meal is safe.

A sample card

This is the layout a kitchen would read. The name says “Sample guest” so it is obvious the foods are an example. The same layout is used for all 38 languages when you use your own card with Plus. Languages we are not highly confident about are marked beta, please double-check, on the card itself.

Example in English

Chef card

Sample guest

Please read this before you prepare food. Please tell the chef.

Must not eat

  • Peanuts — can cause anaphylaxis
  • Mustard — severe allergy

Please check ingredients and cross-contact (cross-contamination) with the kitchen before serving, including shared fryers, sauces, garnishes and boards.

In the UK, call 999 if someone is having a severe allergic reaction.

A communication aid from the guest. Not a medical document, and not a substitute for checking with the kitchen.

Fiche pour la cuisine

Sample guest

Use it with SafePlate Plus

Plus is £4.99 a month. It is what lets you fill the card from your profile, pick a language, show it at the table, or print it. The preview on this page stays available either way.

What the kitchen reads

The card asks them to read it before they prepare food. It lists foods that must not be eaten, with the severity in plain language, and asks them to check shared fryers, sauces, garnishes and boards. The English card says to call 999 in the UK. The French sample says to call 112, the number used across the EU.

It is a communication aid, not a medical document. It does not replace an adrenaline injector or a care plan, and it does not decide that a dish is safe.

How to use your own card

  1. Set up your SafePlate profile with the foods you avoid, including peanut, mustard and anything else, and how severe each one is.
  2. With SafePlate Plus, open the chef card and choose a language the kitchen can read.
  3. Show it or print it before you order, and ask them to take it to the person cooking.
  4. Wait for them to confirm the dish. A card they have not read does not change the plate.

Questions people ask

Can I see the chef card without paying?

Yes. This page shows a sample card, in English and in French, so you can see the layout and the kind of wording a kitchen would read. The sample uses peanut and mustard. It is not your card, and it cannot be filled in, shown as yours, or printed from here.

What does SafePlate Plus add?

Plus is what makes the card usable. You fill it from the foods on your profile, choose one of 38 languages, then show it to the kitchen or print it. A language marked beta, please double-check, still needs a reader of that language to confirm it. Search on SafePlate stays free.

Does the card replace talking to the kitchen?

No. It gives them a clear written list so a spoken explanation is not the only record. Someone still needs to check the dish you are about to eat and tell you what they found. If they cannot, choose something else.

Which languages does the SafePlate chef card use?

The card is available in 38 languages, including major UK travel destinations and widely used world languages. This page previews English and French. In the card itself, any language we are not highly confident about is marked beta, please double-check. Food names without a translation stay in English. Arabic, Hebrew and Urdu are set right to left.

Is the chef card a medical document?

No. It is a communication aid from the guest. It does not replace advice from your doctor or allergist, and it is not a substitute for calling 999 if someone is having a severe allergic reaction in the UK.

This is not medical advice. Confirm the meal with the kitchen even after they have read the card. If you think someone is having anaphylaxis, call 999.
